Adult Day Health Care Program

How can a person apply for the Adult Day Health Care Program You can become a client in our ADHC program by filling out an application and getting a referral from your physician, health care or community agency, or come in on your own.

What happens next? Many times potential clients prefer a trial visit to experience our program firsthand. You will be assessed by professionals on our staff and approval from your physician is required. After the referral and assessment you can officially become part of our ADHC family!

What services and activities are offered by ADHC? Health Monitoring: Licensed nurse on site; administration of medications; direct nursing care; nutrition planning and supervision; therapy programs; health and personal care education.

  • Rehabilitation: Physical, occupational, and speech therapy as needed; treatment for chronic disabilities such as arthritis or for an acute illness such as trauma or stroke. Mobility training; exercise.
  • Foot Care: Provided by a local podiatrist.
  • Social Work Services: Individual and family counseling; caregiver support; special needs counseling.
  • Nutritional Care: Diet planning and counseling; breakfast, snacks and hot lunch provided; special diet planning available.
  • Recreational Activities: Games; music; arts and crafts; fellowship; special performances and visits by community schools; holiday parties and movies.
  • Transportation: Assisted door-through-door service for fragile or disabled passengers including wheelchairs. (Transportation may not be available in all areas.
Who is eligible for ADHC? Any disabled person 18 years or older, or adult 55 or older with physical or mental health conditions which impair active independent living and whose physician prescribes ADHC services. Any individual with walker and wheelchair is welcome.

Who pays for ADHC and how? One low daily charge covers all the services included in the program. Medi-Cal reimbursement is available.

When is the Program open? 8:30 am to 1:30 pm

Office Hours: 8:30 am - 4:00 pm
